Case Management Services

What We Do

Case Management furthers the mission of Catholic Charities West  Virginia, provides comprehensive social services to the poor and vulnerable and confirms the agency’s dedication and commitment to reduce poverty among the people of the Mountain State. The Case Manager and consumer work out a plan for the consumer to achieve goals towards self sufficiency. Within the process, the consumer is supported with guidance and additional services as they face setbacks and successes on the path to realizing their goals.

Available Services

  • Qualifies for medicaid card through the Department of Health & Human Resources
  • Evaluation of Social environment and support needs
  • An individual Service Coordination Plan developed for the specific needs of each client
  • A Case Manager to act as an Advocate and Coordinator of each Client’s care
  • Provide assistance with program enrollment and program information to potential clients
  • Help direct care that is a reflection of each clients needs and wishes
  • Respite for family members who may have been providing all of the clients care
  • Promote community awareness of services available
